How Much Does GLP-1 Medication Cost Without Insurance?

Manufacturer self-pay prices for Wegovy and Zepbound, what's not included, and questions to ask before paying. Last verified October 4, 2026.

GLP-1 prescription medications can have very different monthly costs depending on the product, the dose, and how you pay. This guide summarizes what the manufacturers publish for self-pay (cash) customers, what those prices don't include, and what to ask any provider before you commit.

Short version

  • Manufacturer list prices are high. NovoCare lists a list price of $1,349.02 per package for each Wegovy form (pill, pen, and HD pen).
  • Both manufacturers sell directly to self-pay patients at lower published prices, which depend on the product and dose.
  • A prescription from a licensed provider is required for every option below.
  • Prices usually do not include the provider visit, and injection supplies may be extra.
  • Prices change. Always check the manufacturer's page on the day you buy.

Wegovy (semaglutide) self-pay prices via NovoCare

Per NovoCare's Wegovy cost page, checked October 4, 2026:

Zepbound (tirzepatide) self-pay prices via LillyDirect

Per LillyDirect's Zepbound page, checked October 4, 2026. On that page, the single-dose vial and the KwikPen are listed at the same self-pay price for each dose:

Lilly states that a month is 28 days (4 single-dose vials or 1 KwikPen), that needles and syringes (vials) or pen needles (KwikPen) must be purchased separately, that this is self-pay only and not insurance, and that terms may change at any time. Lilly also says vials are available only through LillyDirect.

What these prices usually don't include

  1. The provider visit. Prices above are for medication only. On its telehealth listing for obesity care, LillyDirect shows independent providers with self-pay visit or membership fees that, on October 4, 2026, ranged from about $29 per visit to $299 per visit, or $99 to $149 per month for membership models. Lilly says it doesn't endorse these providers and that fees may vary.
  2. Injection supplies. Lilly lists needles, syringes, or pen needles as separate purchases for its self-pay products.
  3. Dose increases. Your provider may increase your dose over time, and the price changes with the dose. Ask what you would pay at the dose you'd be taking over the long term, not only the starting dose.
  4. Refill timing. For higher Zepbound doses, the lower price depends on refilling within 45 days.

If you have insurance or Medicare

  • Commercial insurance: NovoCare and Lilly each advertise "as little as $25 per month" savings offers for eligible commercially insured patients if the plan covers the medication. NovoCare notes a maximum savings of $100 per month and excludes government beneficiaries. Coverage depends on your plan.
  • Medicare: Both manufacturers' pages describe a Medicare GLP-1 Bridge program in which eligible Medicare patients pay $50 per month, starting July 1, 2026. NovoCare says eligibility is determined by CMS and that this is not a guarantee of coverage. See CMS's Medicare GLP-1 Bridge page for current rules.
  • HSA/FSA: LillyDirect notes its products may be HSA/FSA eligible, depending on your plan.

Compounded versions

You may see lower advertised prices for compounded semaglutide or tirzepatide.   1. What is the total monthly cost, including the visit, any membership, the medication, and supplies?
  2. Is the medication FDA-approved or compounded?
  3. What will I pay at each dose level?
  4. What are the refill timing rules for any discount?
  5. What are the cancellation and refund terms?

Bottom line

The price you see in an ad is rarely the full monthly cost. Compare the total monthly cost at the dose you're likely to be on, confirm whether the product is FDA-approved, and check the manufacturer's own page on the day you buy.
