GOLEAN HORIZON SMS PROGRAM TERMS & CONDITIONS

1. Program Description
The Golean Horizon SMS program provides transactional and informational text messages related to requests and services initiated by users.
Messages may include requested educational information, quiz results, booking confirmations, appointment reminders, follow-up communications, customer-service messages, and status updates related to a provider review or other service requested by the user.
2. Consent to Receive Messages
By voluntarily selecting the SMS consent checkbox and providing your mobile telephone number through the Golean Horizon website, you agree to receive recurring informational SMS messages from Golean Horizon.
The SMS consent checkbox is optional and is not selected by default.
Providing a telephone number alone does not constitute SMS consent.
SMS consent is not a condition of purchase and is not required to receive information or services.

You must be at least 18 years old to participate in the SMS program.

Golean Horizon does not charge a separate fee for SMS messages. Your wireless carrier may charge you according to your mobile plan.

to a Golean Horizon SMS message.
After you send STOP, you will receive an opt-out confirmation and will not receive further messages from the applicable SMS program unless you provide new consent or re-enroll.

Golean Horizon does not sell, rent, lease, or share your mobile telephone number, mobile information, SMS opt-in data, or messaging consent with third parties or affiliates for their own marketing or promotional purposes.
SMS consent information is not sold, rented, transferred, or shared for marketing or promotional purposes.

9. No Medical Advice Through SMS

10. Prescription and Provider Review Disclaimer
Golean Horizon provides educational information regarding prescription options and the telehealth provider review process.

Golean Horizon does not guarantee eligibility, provider approval, or receipt of any prescription.
